Description
Woodward Resource Center is requesting quotes for services for an Orthotic Clinic to be held one (1) time per month. Clinic would be held on the 3rd Tuesday of every month. These on campus services would include but are not limited to measuring, modification, fitting and repair of orthotic devices, prescription shoes and orthotics, custom bracing such as AFO’s, KAFO’s, consultation with Woodward Resource Center staff, etc.. Clinic would be onsite at Woodward Resource Center, located at 1251 334th Street, Woodward, IA 50276. Interested Vendors will be responsible for Medicare Claims directly. Contractor shall provide WRC with a per clinic rate to hold the outreach clinic at our campus. This will also result in a service contract. Interested vendors shall contact the purchasing agent listed for an example of our service contract. Background checks, licensing, and proof of insurance will be required of the awarded contractor. Questions must be received no later than end of business day, 4:00 pm, CST, February 6th, 2024. We will accept quotes through end of business day Thursday February 8th, 4:00 pm, CST. Quotes must be received by email at jfrey@dhs,state.ia.us
