Opioid Use, Prescription Management and Prevention Education for Iowa Healthcare Providers

Description
The purpose of this Request for Proposal (RFP) # 58825019 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to provide training and information to healthcare providers and patients on strategies to reduce or eliminate the prescribing of opioid medications for pain management after surgery.
Beginning in the late 1990’s, pain was recognized as a public health issue, often called “The 5th Vital Sign”. With this increased focus on pain, physicians began to prescribe opioid medications for pain relief with the promise of pharmaceutical companies that these medications were not addictive. These medications began to be diverted and misused. It became clear that opioid medications are highly addictive. As a result, opioid overdoses began to increase dramatically.
The opioid crisis is devastating to the victims, their families, and the healthcare system. Nationwide, the effect is seen in both rural and urban areas. In 2023, CDC reported an estimated 81,083 overdose deaths involving opioids nationwide. While this number is steadily decreasing, the rise of synthetic opioids remains a major concern. In Iowa, the rate of opioid prescriptions has steadily decreased since 2015 but opioids remain involved in 54.9% of overdose deaths according to CDC.
The opioid epidemic has led to an increased awareness of the risks of opioid use among both health professionals and the public. A prominent academic detailing strategy has been to focus on promoting opioid prescribing practices that are consistent with such guidelines (e.g., 2022 CDC Clinical Practice Guideline for Prescribing Opioids for Pain) or those created by states, localities, or professional organizations. Despite reductions, opioid stewardship remains an important preventative measure.
United states surgeons are among the highest prescribers of opioids in comparison to surgeons abroad. While surgical procedures are known to result in post-surgical pain, opioid prescriptions often exceed the patient need or length of expected recovery. This is an unintended reason for misuse or dispersion of opioids including patients becoming inadvertently reliant on opioids after pain goes away.
Ongoing efforts for prevention and recovery have been present in Iowa. The State of Iowa is working to address this crisis by investing in strategies for opioid use prevention and recovery. This included funding a $1 million education initiative for Iowa Healthcare providers and patients with a focus on non-opioid post-surgical pain management.
