Description

All vaccine will be ordered or confirmed by written purchase orders, each citing as a minimum the following:
(1) Date of Order
(2) Contract Number and Order Number
(3) Item Description, Quantity and Unit Price
(4) Delivery or Performance Date
(5) Place of Delivery or Performance (Including Consignee)
(6) Packaging, Packing, and Shipping Instructions, if any
(7) Accounting and Appropriation Data
(8) Statement to indicate if partial deliveries are not acceptable
(Lack of a statement shall be construed to mean partial deliveries are acceptable and payment shall be made as
required elsewhere herein)
(9) Any Other Pertinent Data
DELIVERY WITHIN 15 DAYS FROM DATE OF ORDER. No Returns. Payment: NET30
